10/18/2011 - The IPRO 10th Scope of Work Care Transitions Overview teleconference learning session was the launch of the IPRO Learning and Action Network educational series. The teleconference presentation provides:

  • An overview of the Centers for Medicare and Medicaid Services (CMS) 10th Scope of Work Care Transitions Initiative goals, objectives and program opportunities for cross-setting collaboration to improve the patient/resident/family/caregiver experience and provider communication and care coordination across the health care continuum.
  • Review of the IPRO 9th Scope of Work Care Transitions Initiative (August 2008 - July 2011) structure, interventions and outcomes
  • Sharing from hospital, home health and skilled nursing facility champions on their experience with care transitions efforts
  • An overview of the evidence based strategies and interventions targeting readmission drivers
  • Strategies to engage patients/residents/caregivers in their health management

The Outcomes Congress is a learning conference for health care professionals vested in improving person-centered transitions and care across the health care continuum. Materials from the IPRO Care Transitions Learning Collaborative

The Care Transitions Learning Collaborative, organized by IPRO, took place over an 18-month timeframe from November 2008 through April 2010. The collaborative was established to provide a venue for the transition area health care providers to share individual setting specific challenges and encourage cross-setting partnerships and initiatives.
