Healthcare Professionals

IPRO is working with a wide range of healthcare providers in multiple settings of care to improve the quality and effectiveness of care transitions. These settings include, but are not limited to, hospitals, skilled nursing facilities, home health agencies, physician practices, dialysis centers and hospice organizations.

How We Work with Healthcare Professionals

IPRO built the foundation for our Care Transitions project on bringing healthcare providers from various settings together to evaluate existing transitional care systems and processes. We then emphasized building partnerships and collaboration among this diverse community to prioritize improved communication and care coordination.

IPRO experts met onsite at physician practices in the region, interviewing physicians, nurses, practice managers and support staff to gain insight into the challenges they encounter assisting their patients during transitions of care. Likewise, we have sought feedback from other healthcare providers, as well as from Medicare beneficiaries themselves.

We are continually developing and sharing tools and strategies to improve communication between hospitals, primary care physicians and specialists working in the community.

Care Transitions Tools & Resources

While direct participation in our project is limited to the five New York counties in which our project is focused, IPRO and its partners have developed and compiled a wide array of tools, resources and information that is available to any healthcare professional or community partner interested in working to improve care transitions. Please visit our Tools & Resources page to further investigate improvement strategies and see what role you can play in improving care transitions for your patients.
