Target Audience

 Nephrologists; Physicians Assistants; Nurse Practitioners and Nurses

Objectives

  1. Discuss the prevalence and negative impact of medication errors encountered during care transitions
  2. Identify and discuss key data elements required for successful medication reconciliation.
  3. Present a systematic approach to ensure accurate medication reconciliation for all patients.

Accreditation Statement

This activity has been submitted to the American Nephrology Nurses Association (ANNA) for approval to award contact hours. The American Nephrology Nurses Association (ANNA) is accredited as an approver of continuing nursing education by the American Nurses Credentialing Center’s Commission on Accreditation.
