Five Diamond Approach to Patient Safety Program

Program Overview

IPRO End Stage Renal Disease (ESRD) Network of New York believes it is important for each dialysis facility to incorporate patient safety into their organizational culture. To help promote patient safety values, IPRO ESRD Network is implementing the 5 Diamond Approach to Patient Safety Program. The goal of this program is to help dialysis facilities better implement patient safety principles among both staff and patients.

This interactive program is designed so that each dialysis facility that completes one module and submits documentation (i.e. write up about how you implemented the module and the outcome achieved) to the ESRD Network, will receive one "diamond", with a maximum of 5 diamonds awarded to a facility within the participation period. All participating facilities that receive at least 1 diamond, will be recognized as follows:

1 - 4 Diamond Facilities
Facility accomplishment written in Network Notes newsletter Facility recognized on the Network's website

5 Diamond Facilities
Five Diamond Facilities will receive a recognition letter and a plaque recognizing the facility's Five-Diamond Patient Safety Program status.

Upon successful completion of five modules, we encourage you to market your facility as a Five-Diamond Patient Safety Facility.

The following schedule applies to Five-Diamond Status:
Achievement between 1/1/2011 and 1/1/2012 is valid until 1/1/2013.

Providers may renew their diamond status for another year by completing two additional modules.

If you are interested in your facility participating in our new 5 Diamond Approach to Patient Safety program, please fill an application, and submit it to our office as soon as possible. Upon receipt, the Network will register your application and planned activities and you may begin the program. You must complete the mandatory Patient Safety Principles module first, and then you may complete additional modules in any order.

There is no charge to participate and you may complete as few modules as you like. All participants who complete at least one module will be recognized!
