Healthcare Professionals

In our role as the Medicare Quality Improvement Organization for New York, IPRO works collaboratively with physician offices, other healthcare providers and partners in the renal community to improve care for chronic kidney disease patients. IPRO quality improvement team has adopted the Chronic Care Model as a foundation for our work to improve care for CKD patients. Our focus is on promoting physician awareness and implementation of preventive CKD care processes across New York State.

How IPRO Works with Healthcare Providers

We have recruited and are working closely with a number of primary care physician practices with which we are testing clinical interventions to reach at-risk populations. In addition, we coordinate CKD educational sessions and assemble educational materials that help emphasize the importance of identifying and treating CKD for healthcare providers and patients across the state.
