Ambulatory care

As part of our Medicare Quality Improvement Organization work IPRO assists ambulatory care practices in:

  1. Improving Health for Populations and Communities.
    • Using their electronic health record (EHR) systems to coordinate preventive services (including mammography, colorectal screening and immunizations) and report related quality measures to the Centers for Medicare & Medicaid Services (CMS).
    • Reducing patient risk factors for cardiac disease.
    • In partnership with New York State's Health Information Regional Extension Centers (RECs) -- NY eHealth Collaborative and the New York City Regional Electronic Adoption Center for Health (REACH) IPRO is providing education and technical assistance to support integration of health information technology into clinical practice.
  2. Preventing Oral Anticoagulation Adverse Events
    • Improving health outcomes and reducing adverse drug events among high-risk populations, by providing additional technical support to participants in the national Patient Safety and Clinical Pharmacy Services Collaborative.
  3. Integrating Care for Populations and Communities (Care Transitions)Â
    • Improving the safety of care transitions, through re-designed workflows, to ensure patients recently discharged from a hospital receive timely follow-up care from their primary providers.
  4. Beneficiary- and Family-Centered Care
    • Respond to Medicare beneficiary complaints.
