Title: ESRD Network: Syracuse Regional Patient Education Meeting
Description:

This will be an afternoon meeting to present: Patient roles and responsibilities, sharing important health information and discussing many pertinent issues affecting dialysis patients today; such as: phosphorous management, vascular access, what is the ESRD Network, vocational rehabilitation, exercise and much more.

Patient Representatives (PAC REPS): Attendance at the Annual Meeting is a requirement for registered facility PAC Reps. Please encourage PAC Representatives to attend and provide any assistance you can to help them to attend. Please encourage all of your facility PAC Reps and patients to attend this very important meeting.

Social Workers/Administrators: If your facility does not have a patient representative, this is an excellent opportunity for patients to learn about what it takes to be a patient representative, who the IPRO ESRD Network is and how we can help.
